Gli acufeni sono definiti come una percezione uditiva in assenza di una sorgente sonora reale esterna all’organismo, possono essere riferiti come ronzio, fischio, sibilo, fruscio ecc….
Possono essere percepiti in un solo orecchio o in entrambi, o all’interno della testa. L’intensità degli acufeni è molto variabile e in alcuni casi sono talmente intensi da impedire il sonno notturno. Si percepiscono nettamente meglio in un ambiente silenzioso, in assenza di rumori di fondo.
Si tratta in genere di rumori di origine vascolare, tubarica o muscolare trasmessi per via ossea ed in grado di stimolare i recettori del suono. L’origine degli acufeni è di difficile definizione perchè costituiscono un sintomo e non una malattia e tuttora non è possibile una valutazione attendibile. L’incidenza del sintomo acufene è rilevante ed è variabile, a seconda delle casistiche, dal 10 al 17%. Da ciò deriva che in Italia ne sono interessati più di 5 milioni di soggetti. La gravità del sintomo non dipende tanto dalla sua intensità, ma soprattutto dalla reazione di fastidio che esso genera. L’acufene è tanto più fastidioso quanto più viene percepito dal paziente come una minaccia per la propria salute fisica. Molti pazienti temono che l’acufene sia un campanello d’allarme per gravi patologie a livello cerebrale, oppure che possa condurre alla sordità. Questa è invece una paura infondata. Così, come gli animali indirizzano principalmente la loro attenzione uditiva sui rumori generati dai predatori pericolosi, così coloro, che considerano l’acufene una grave minaccia o un allarme, non sono in grado di distogliere l’attenzione da esso. Entrando più nel dettaglio, gli acufeni possono essere provocati da patologie dell’orecchio esterno come un banale tappo di cerume e sono quindi facilmente risolvibili, altre cause possono essere otiti medie catarrali, patologie infiammatorie a carico della tuba di Eustachio (un canale che mette in comunicazione l’orecchio medio con la parte posteriore delle cavità nasali), stenosi carotidee, ipertensione arteriosa, diabete, abuso di alcol e caffeina. Una causa specifica degli acufeni è di pertinenza maxillo facciale e consiste in un problema a carico della articolazione temporo mandibolare, spesso, in questi casi, agli acufeni si associano altri sintomi come: nausea, cefalea, dolori cervicali, vertigini e dolore auricolare. per la soluzione di questi specifici acufeni è spesso sufficiente l’applicazione di un bite gnatologico durante la notte, in altri casi è necessario un intervento di chirurgia maxillo facciale. Ma nella maggior parte dei casi non è possibile trovare una causa evidente degli acufeni, per cui si parla di acufene soggettivo idiopatico. In considerazione delle difficoltà, in molti casi, di individuare una sicura origine dell’acufene, soprattutto negli ultimi anni, si è data molta importanza alla sua percezione a livello della corteccia uditiva. Non esiste un farmaco risolutivo per la terapia degli acufeni. Sono in commercio molti integratori, soprattutto a base di Ginko biloba, binacospino e valeriana, che possono ridurre l’entità del sintomo, in quanto hanno un lieve effetto sedativo nei confronti dell’ansia alla base dell’accentuazione del sintomo. Esiste poi una terapia non farmacologica che può avere effetti positivi: la terapia di riprogrammazione dell’acufene. Questa terapia si basa su una stimolazione sonora Alla quale il paziente deve essere sottoposto per la maggior parte della giornata. La stimolazione sonora non crea fastidio al paziente ed ha la funzione di distrarre l’attenzione dall’acufene, senza coprirlo. Viene erogata da minuscoli erogatori di suono indossabili ed esteticamente non invasivi.
La stimolazione sonora non interferisce con le normali attività quotidiane in quanto è molto lieve.
Quando l’acufene ha perso il suo significato di negatività e pericolosità, inizia a diminuire fino, in alcuni casi, a non essere più percepito.
