La fibrosi cistica è la malattia genetica grave più diffusa ,colpisce le ghiandole esocrine, come quelle che producono muco e sudore. Il malfunzionamento di tali ghiandole si traduce in gravi alterazioni a carico dei polmoni, pancreas, fegato, intestino, seni paranasali e apparato riproduttivo.
La causa primaria della malattia risiede nella mutazione del gene CFTR.
Un italiano su 25 è portatore del gene mutato. Perchè la malattia si sviluppi è necessario che entrambi i genitori siano portatori del gene mutato.
Circa una coppia su 600 è composta da due portatori sani del gene CFTR mutato
e a ogni gravidanza ha il 25% di probabilità di avere un bambino malato di fibrosi cistica.
La fibrosi cistica provoca una produzione di muco più spesso del normale che, a sua volta, porta ad ostruzioni nei due apparati respiratorio e digerente; nell’apparato respiratorio il muco va ad ostruire le vie aeree con conseguenti infezioni polmonari recidivanti, mentre nell’apparato digerente ostruisce i dotti biliari e pancreatici,provocando gravi alterazioni della digestione.
I sintomi della fibrosi cistica non compaiono tutti insieme dalla nascita ed inoltre cambiano, come gravità da paziente a paziente.
Nei bambini un primo segnale è rappresentato da un sapore particolarmente salato del sudore, in quanto più ricco di sodio, o una stitichezza particolarmente persistente fin dalla nascita con possibile occlusione intestinale.
Successivamente, con il passar del tempo compaiono altri sintomi e alterazioni di apparati. A livello dell’apparato respiratorio il muco più denso e viscoso ostruisce le vie respiratorie provocando infezioni ricorrenti (bronchiti e polmoniti) che diventano sempre più resistenti agli antibiotici. Frequenti sono anche le sinusiti ed i polipi nasali. Più avanti nel tempo compaiono anche le bronchiectasie (dilatazioni dei bronchi) con accumulo di muco al loro interno.
A livello dell’apparato digerente compaiono dolori addominali crampiformi, diarrea con emissione di feci dall’aspetto oleoso alternati a periodi si stitichezza persistente. La bile ed i succhi pancreatici diventano anch’essi molto densi e si accumulano,ciò impedisce un corretto assorbimento di principi nutritivi,soprattutto vitamine liposolubili (A.D.E.K) Frequenti sono quindi le pancreatiti e la formazione di calcoli della colecisti con conseguenti colecistiti.
Il malassorbimento provoca un ridotto accrescimento in termini di altezza e peso.
Oggi la grande maggioranza dei casi di fibrosi cistica è individuata alla nascita grazie a un esame del sangue al quale sono sottoposti tutti i neonati. Lo screening neonatale si esegue misurando nel sangue, entro 48-72 ore dalla nascita, la concentrazione di tripsina. Se questa è alta, si procede ad un secondo esame, tra il quindicesimo e il ventesimo giorno di vita, presso un Centro dedicato. Questa seconda prova serve a confermare la diagnosi. L’individuazione precoce della malattia consente l’avvio tempestivo delle cure e ciò migliora l’accrescimento, rallenta lo sviluppo dei danni alle vie respiratorie e, in generale, migliora l’andamento della malattia. Lo screening neonatale è obbligatorio in tutte le Regioni italiane. Tramite un’amniocentesi può anche essere eseguita una diagnosi prenatale. Non esiste una terapia specifica e risolutiva della fibrosi cistica.
Molto utili le metodiche per favorire l’eliminazione del muco che comprendono la fisioterapia toracica (da effettuare almeno due volte al giorno), il drenaggio posturale, la percussione del torace e l’impiego di dispositivi particolari che facilitano l’espettorazione. In questi pazienti viene inoltre consigliata un’attività fisica regolare, che stimola l’espulsione del muco. Nelle diverse fasi della malattia potranno essere impiegati antibiotici (anche per aerosol) a scopo terapeutico e preventivo, cortisone e broncodilatatori. Nei casi più gravi può essere preso in considerazione il trapianto di polmone.
A causa della difficoltà all’assorbimento delle sostanze nutritive e del dispendio energetico determinato dagli sforzi per la respirazione, nei pazienti con fibrosi cistica è necessaria una dieta ipercalorica ben bilanciata con la supplementazione di vitamine (in particolare di tipo A, D, E, e K). È utile inoltre l’assunzione di enzimi pancreatici ed eventualmente di lassativi come il lattulosio, utili per prevenire la stipsi e le ostruzioni intestinali. Fondamentale una costante e abbondante idratazione.
